Testosterone deficiency, also known as hypogonadism, is a condition in which the testes are unable to produce enough testosterone to fulfill the body’s needs. Testosterone deficiency has many possible causes, including genetic abnormalities, injury to the testes, and being on certain medications. Normal aging also may play a role in the decline of male testosterone levels. It is also known as low testosterone.

The testes produce testosterone regulated by a complex chain of signals that begins in the brain. This chain is called the hypothalamic-pituitary-gonadal axis. The hypothalamus secretes gonadotropin-releasing hormone (GnRH) to the pituitary gland in spurts, which trigger the secretion of leutenizing hormone (LH) from the pituitary gland. This hormone stimulates the Leydig cells of the testes to produce testosterone. Normally, the testes produce 4-7 milligrams (mg) of testosterone each and every day.

After puberty, testosterone production increases rapidly, and will decrease rapidly after age 50. Recent estimates show that approximately 13 million men in the United States experience testosterone deficiency and less than 10-percent receive treatment for the condition, which is growing in cultural acceptability.

Studies also have shown that some men with obesity, diabetes, or hypertension may be twice as likely to have low testosterone levels, though as stated, low testosterone and testosterone deficiency can be caused by taking certain medications, chemotherapy, infections and other basic causes.

Signs of testosterone deficiency depend on the age of onset and the duration of hormonal deficiency. Congenital testosterone deficiency is usually characterized by underdeveloped genitalia, and sometimes even undeterminable genitalia. Acquired testosterone deficiency that develops near puberty can result in enlargement of breast tissue (gynecomastia), sparse or absent pubic and body hair, and underdeveloped penis, testes, and muscle. Adults may experience diminished libido, erectile dysfunction, muscle weakness, hair loss, depression, and other common mood disorders.

PROZAC: PROBLEMS WITH INTERACTIONS

People who are prescribed the new antidepressant drug Prozac should be aware of reports of potentially fatal toxic interactions with tricyclic antidepressants (American Journal of Psychiatry, November 1988). Another interaction, this one involving severe lithium toxicity in a person taking both lithium and Prozac, has also been reported (American Journal of Psychiatry, February 1989).

Prozac is the most prescribed antidepressant for depressed adults. Although it is used in children and adolescents, until recently good studies that demonstrate its effectiveness have been lacking.

In a recent study, depressed children seven to 17 years old were treated either with Prozac or a placebo. These 100 children were seen weekly for eight weeks.

In the acute phase of depression, 56% of the children receiving the Prozac improved, while only 33% of those receiving the placebo improved. Of those who completed the entire eight-week study, 74% of those treated with Prozac had long term improvement, while only 58% of the placebo group did. Complete remission was rare: 31% of the treatment group versus 23% of the placebo group.

Write-offs on bad investments and severance packages for layoffs trimmed Exelon Corp.’s fourth-quarter profits by 31 percent.

The profit news came a day after Exelon, the parent company of Commonwealth Edison Co., raised its quarterly dividend to 55 cents from 50 cents, payable March 10 to shareholders of record Feb. 15, and approved a 2-for-1 stock split. The increased dividend, Exelon CEO John Rowe told analysts on a conference call, is a sign “we are confident in the future earnings strength of this company.”

Exelon’s stock rose $1.13 per share to $66.28, a 1.7 percent increase. It has gained 36.3 percent in the last year, compared with a 27.3 percent return in the Standard & Poor’s 500 Electric Utilities Index.

SPRINGFIELD — Weeks after Exelon Corp. reported a windfall profit, House Speaker Michael Madigan proposed Thursday taxing electricity producers to provide consumers with utility rate relief.

The Southwest Side Democrat offered the twist on stalemated rate- freeze deliberations at a closed-door meeting with House Democrats.

Commonwealth Edison and Ameren Corp. are battling to stave off attempts to extend a 10-year rate freeze that expired last December.

The utilities have argued a rate freeze would bankrupt them, but the idea floated Thursday would target the companies those utilities buy power from — including ComEd’s corporate parent, Exelon.

In the late eighties and early-nineties, Prozac was pronounced as the new wonder drug, a magical cure for depression that was patronized by millions of people worldwide. Prozac was later discredited when murmurs spread that it could spur suicidal tendencies in regular users. These days, Prozac is making a big comeback in the most unlikely of places: law enforcement.

Online news reports last June 25 graphically detailed the latest plan of the United Kingdom to control sex offenders - feed them with Prozac until they’re calm as lambs. It seems the British government, impressed with Prozac’s calming effects, plans to administer the controversial drug to jailed pedophiles and other sex offenders. In doing this, they hope to tame the beastly nature of the inmates and shackles their libidos so they do not commit any more heinous crimes.

The plan calls for 100 inmates from nine different prisons to serve as the first, recipients of Prozac in the British penal system. If the plan works, British authorities say they will proceed to administering Prozac to as many as 10% of the prison population and see if it’ll calm them down.

The plan is a rather unique way to approach the problem of sex offenders. You might say it’s like castration on a mental and emotional level. You could also say that the plan is not morally ethical considering that it is tantamount to drugging the prisoners into submission, if not immobility. And then, there’s also the controversial history of the drug to consider.

Prozac was heaped upon the American public in 1988 and was an immediate smash hit. Millions flocked to drugstores to purchase the new drug, which seemingly had touched a raw nerve of depression that lay latent not only in America but in the rest of the world. It became cool to ingest Prozac. There is no doubt that the aggressive and creative marketing communications campaign that promoted Prozac was a tremendous boost. Sales hit the roof as the drug earned billions of dollars in sales annually.

Then, in 1990, Prozac came under severe attack stemming from allegations that Prozac made people preoccupied with thoughts of suicide.

Eli Lilly and Company, makers of Prozac launched its own media campaign citing studies that refuted the connection between Prozac and suicide. Nothing was ever proven in court nor were there any settlements, But the damage had been done and Prozac’s reputation was tarnished.

Let’s see if the British government can find some use for the drug that will help us all sleep a little better.

There are no adequate and well controlled studies with venlafaxine in pregnant women. Venlafaxine is not approved for the treatment of depressive phases of bipolar disorder; this has some potential danger as venlafaxine can induce mania, mixed states, rapid cycling and/or psychosis in some bipolar patients, particularly if they are not also being treated with a mood stabilizer.[2] Venlafaxine is perhaps one of the most likely of all modern antidepressants to trigger manic and hypomanic states.[citation needed]

AstraZeneca has submitted a New Drug Application for a sustained release version of quetiapine in the United States, Canada, and the European Union in the second half of 2006 for treatment of schizophrenia.[10][11] AstraZeneca will retain the exclusive right to market sustained release quetiapine until year 2017.
